Field Trip Permission Form

I have included a copy of the Ocean View Field Trip Permission form for anyone who might need one at the last minute.  We are not permitted to take anyone off campus without this form signed by the parent or guardian.  We appreciate it if you could sent this back as soon as possible to avoid anyone being excluded from a field trip.  This can be traumatic for students and teachers! 

Please feel free to print a copy of this permission slip if you need one.
